ALARM BRIEF
De heer / mevrouw geboren
heeft niet of onvoldoende functionerende bijnieren
ten gevolge van
Hij/zij gebruikt daarvoor dagelijks:
hydrocortison / cortison acetaat in een dosis van mg
Bij stress-situaties (koorts, infecties, ongeval) dient hij / zij de dosis hydrocortison / cortison acetaat tenminste te verdubbelen en vervolgens voor nadere instructies met de behandelend arts contact op te nemen.
Hij / zij beschikt over 1 ampul Solu-Cortef (=100 mg hydrocortison) welke in noodsituaties (braken, coma) i.m. of i.v. dient te worden toegediend. Aansluitend dient contact te worden opgenomen met de behandelend specialist.

He / she is being treated with (daily dose):
hydrocortisone / cortisone acetate mg
He / she has been instructed to at least double the dose of hydrocortisone / cortisone acetate during episodes of stress (fever, infection, trauma) and to contact his / her physician.
He/she is in possession of 1 ampoule of Solu-Cortef (= 100 mg of hydrocortisone) which has to be administered i.m. or i.v. in case of emergency (vomiting, coma). Thereafter his / her endocrinologist should be notified.
